Emergency Medical Authorization

It's vital that we have current medical information on each band student.  Even though you have submitted it to the school, we're not able to pull those every time we travel to away games, parades or band shows, so we need our own copies to take with us.  There have been instances where we have had to seek medical care and had to refer to the form.  The form is available here as a downloadable PDF and can be e-mailed to either Mrs. Moniaci or the Boosters, who will forward to her.
